Home Health Care




If you are experiencing an illness or have just been released from the hospital, extended care facility or nursing home, the Memorial Home Health Care team is at your service. We’ve provided quality home health care to patients ages 14 and up in Union County and the surrounding area since 1978.


We are accredited by The Joint Commission to provide medical care in the home, in addition to being Medicare certified. Since 2006, our staff has been recognized as one of the top 25% home health agencies in the nation.

Our professional, compassionate staff provides nursing, physical and speech therapies, dietary services, personal care (bathing, dressing) and light housekeeping services. A nurse is on call 24 hours a day, seven days a week to assist you. Turn to us if you need help with one or more of the following:

  • Disease education and care management
  • Physical assessment
  • Post-surgery and post-hospital care
  • Medication instruction and management
  • Blood pressure monitoring
  • Wound care
  • Diabetic care management
  • Ostomy care
  • Infusion therapy for antibiotics and some chemotherapy
  • Injections
  • Enteral feedings
  • Physical strengthening or ambulatory problems
  • Speech and cognitive (memory) problems
  • Dietary evaluation and management
  • Community resources
  • Short-term counseling and education for financial assistance

Memorial Home Health Care accepts Medicare, Medicaid and most private insurances. We can assist you in determining your insurance coverage. We stand ready to bring our expertise to your door.

Requirements:
